Explain whether speaker labels are reliable. A transcript may contain recognition errors, overlapping speech, or informal proposals that were never accepted. Treat extracted actions as candidates for confirmation rather than a definitive record of what everyone agreed to do.

Request a constrained output

Try this original prompt: “Extract explicitly agreed action items from these notes. Return action, owner, due date, and supporting excerpt. Use not stated when an owner or date is absent. Put suggestions and unresolved questions in separate sections. Do not assign work or convert tentative language into a commitment.” A source excerpt makes each candidate easier to verify without rereading the entire meeting.

Resolve ambiguity with people

Suppose the notes say, “We should ask finance about the revised estimate.” That may describe an idea without assigning responsibility. Confirm the action with the meeting owner rather than accepting a model-generated owner. Also check relative dates such as “next Friday” against the meeting date and intended time zone. Preserve ambiguity until someone with the appropriate context resolves it.

Separate decisions from tasks

A decision records a choice; an action records work someone must perform. The same conversation may contain both, but they belong in different tracking structures. Use the project decision log for accepted choices and their rationale. Keep unresolved questions visible so they do not disappear when the action list is shortened for distribution.

Confirm before updating systems

Share the candidate list with the responsible reviewer, then record confirmed actions in the agreed tracker.
